What is Needed
for Registration
Dear Parent:
Thank you for your interest in the First Baptist Head Start Program!
Please be sure to fill out following forms, you can find them below in word or just click to open:

Once you have filled out forms please call to set up an appointment with a Family Service representative and plan to be here approximately 30-40 minutes. The Program will also require for your child to have a Physical and
Dental examinations, we will give you the forms here at the time of your appointment.
If you choose to e-mail your applications, your date and time of your appointment will be e-mail back to you.
In order to start processing your application we will need from you the following documents:
· Parents Income
· Child’s Birth Verification
· Child’s Immunization Card
· Health and Dental Insurance Name (if any)
· Health and Dental Insurance Number (if any)
· Name of Doctor and Dentist, address and phone #’s
When you have this forms fill out, please call the Office and set up an appointment. The number to call is:
(925) 473-2000. Office hours: Monday - Friday, from 7:30 a.m. - 6: 00 p.m.
If for some reason you can not keep this appointment or you are running late, 15 minutes or more, you will be ask to reschedule for another appointment. Please call to reschedule your appointment.
